Client situation: A large CCRC was struggling with RUG levels and LOS as well as a need to increase programming in the Assisted Living Facility (ALF) and Independent (I) living population.

Solution: to implement management tools to increase RUG levels while improving outcomes and managing the LOS at the same time introducing new services to capture the ALF and I patients that were seeking services elsewhere.

Upon our arrival at the facility we discovered a passive approach towards managing the caseload. Two days prior to the contract being start up; we personally addressed each patients care plan and modified our approach to maximize the patient's outcome. Consequently, our initial caseload ramped up RUGs levels dramatically. We also implemented our Casamba software which reduced paperwork and increase efficiencies. Using Casamba's tools we were also able to communicate information to the facility more accurately and timely. By delivering the appropriate care to the patient our length of stay (LOS) stabilized at 37 days and our return to prior level of function increased to 82%.

In an effort to provide Assisted Living and Independent Living residence the service they demanded, we brought in experts to address their needs. We introduced a true outpatient therapy clinic with a focus on balance, strengthening, and overall wellness. This is when we developed the "Wellness Strategies Program". We identified areas that the facility and patient could benefit from to maximize the residence independence. The following programs were created: Aquatics (utilizing the state of the art swim spa), Tia Chi, Personal Training, Yoga, Pilates, and Massage therapy (employing 2 full time massage therapists). By adding these programs the facility realized over a 200% increase in Part B and outpatient revenue.

Five years later we continue to bring new and innovative programs to this facility.
